Why Oral Peptide Delivery Is Moving Beyond the Molecule Itself

Enprotect. IL Capsugel product page describes a ready-to-use enteric capsule using HPMC and HPMC-AS polymers with a Coni-Snap secure closure, protecting acid-sensitive APIs and allowing targeted intestinal delivery. The stated acid resistance is up to pH 6.0, and the capsule eliminates the need for post-filling coating. That is a dosage-form specification: it says where the shell is designed to survive, not what happens to a peptide after release.

Bilayer capsules for ileal targeting. IL Capsugel knowledge-center summary of Millet et al., European Journal of Pharmaceutical Sciences 2026;224:107601, describes a system combining an HPMC- and pullulan-based inner layer with an outer ethylcellulose layer containing sodium alginate as a pore former — diffusion control plus environmentally responsive function in one dosage form. The study evaluated how formulation composition and manufacturing parameters affect gastric resistance, permeabilità, disintegration and release, and identified specific alginate concentrations and curing conditions that produced delayed-release profiles consistent with distal ileum targeting. It is a process-and-formulation parameter study. It does not report in vivo absorption.

Why Oral Peptide Delivery Is Moving Beyond the Molecule Itself

Two-tier GLP-1 protection. IL Frontiers paper on a lipid formulation plus enteric capsules is the most directly peptide-relevant of the three. The authors formulated the GLP-1 analog exenatide as a hydrophobic ion pair (HIP) in a lipid-based formulation with permeation-enhancing excipients, then engineered customized gelatin/HPMC-AS enteric capsules that met USP delayed-release requirements. The capsule shell protects against gastric conditions; the lipid droplets protect against intestinal proteases. The authors describe the result as an in vitro proof of concept and state plainly that in vivo pharmacokinetic and pharmacodynamic studies, extended stability, and long-term formulation–capsule compatibility work remain to be done.

Leggere insieme, the three sources cover three different layers of the same problem — shell, matrice, and peptide-in-vehicle — and none of them demonstrates improved oral bioavailability. That is the correct scope to carry into any internal discussion.

Why the Sequence Alone Cannot Decide Oral Feasibility

UN 2026 Frontiers review of oral peptide barriers states the physicochemical case compactly: peptides are relatively large, polar and charged, which produces low permeability through the mucus layer and the underlying epithelium; they are susceptible to gastric and intestinal proteases; they tend to aggregate; and they are often unstable in ways that complicate robust solid dosage form development. The review puts inherent oral bioavailability for most compounds below 1%.

Those properties are sequence-level facts, and they propagate. Net charge and charge distribution determine whether a peptide will cross a negatively charged mucus network or bind to it. Hydrophobicity sets both membrane affinity and the risk of self-association at formulation-relevant concentrations. Protease-susceptibility motifs — dibasic sites for trypsin-like enzymes, aromatic residues for chymotrypsin, N-terminal residues for aminopeptidases — determine which luminal enzymes will attack first and where a stabilizing modification earns the most. Aggregation propensity determines whether the concentration needed for dosing is even reachable in the chosen vehicle.

This is why a clean certificate of analysis cannot settle oral feasibility. HPLC purity is a relative area percentage; it tells you how much of the UV-absorbing material is the main peak. It does not tell you the counterion form, the residual water, the net peptide content, or how the molecule behaves when it meets gastric acid, bile salts and pancreatic enzymes in sequence. Un peptide può esserlo 98% pure and still fail at the first barrier.

The review also explains why the field has not stalled despite sub-1% bioavailability. Structural modifications that raise potency, metabolic stability and half-life allow therapeutic effect at low single-digit exposure. Oral semaglutide is the established example; the review also cites the macrocyclic PCSK9 inhibitor enlicitide and the IL-23 receptor antagonist icotrokinra as late-stage or clinically successful cases, and Chugai’s cyclic peptide Luna18, which reached 21%–47% oral bioavailability in preclinical models. The threshold moved because the molecule was engineered for it — and that engineering is a synthesis decision.

Protection Has to Be Layered, Not Singular

The two-tier GLP-1 study is worth reading closely because it shows the layers being stacked deliberately rather than discovered by accident.

Unformulated exenatide degraded completely within minutes; the hydrophobic ion pair in lipid-based formulation retained roughly 80%. Fonte: Dumont et al., Front. Drug Deliv. 2026;6:1770094.

At the molecular level, the authors converted exenatide acetate into a hydrophobic ion pair. Log P measurements moved from −2.9 ± 0.3 for the acetate to 0.1 ± 0.2 E 0.9 ± 0.2 for the SLS and DOC ion pairs at a 1:4 molar ratio, and the DOC pair was carried forward. That is a chemistry decision made to change the molecule’s partitioning behavior — the kind of choice that belongs in the synthesis and salt-selection phase, not in a formulation group working from a fixed API.

At the formulation level, the HIP was loaded into a lipid-based formulation with permeation-enhancing excipients. The lipid phase does two jobs: it keeps the peptide out of direct contact with intestinal proteases, and it co-localizes the peptide with the permeation enhancer at the epithelial surface. IL barrier review notes that co-localization is exactly where many oral dosage forms fail — if release occurs in the wrong compartment, or the formulation dilutes, or the enhancer separates from the API, translatability collapses.

At the dosage-form level, the customized enteric capsule provides gastric protection and prevents premature dilution of the lipid phase in gastric fluid, which would otherwise displace the peptide from its protective environment. Release of peptide and enhancer is coordinated at the intestinal absorption site.

The most instructive observation in the paper is a formulation finding that only becomes visible when peptide and vehicle are studied together: short-term stability showed formulation-dependent degradation, with higher impurity formation for solubilized EXE-HIP than for suspended EXE acetate, particularly at room temperature. The physical state of the API — solution versus suspension — changed the degradation trajectory. A team that had characterized only the lyophilized powder would have missed this entirely, and would have discovered it later, at a much more expensive stage.

The authors also flag an oxidation risk grounded in the sequence: metionina 14 and tryptophan 25 are susceptible to oxidation in lipid matrices containing peroxides, aldehydes or metal ions. Their cyclic voltammetry method could not identify the most effective antioxidant, so they recommend fresh lipid excipients and control of light and oxygen exposure instead. That is a concrete example of a sequence feature driving a formulation and handling requirement.

Terminal-Ileum Targeting: Why Site and Transit Matter

The distal ileum is a meaningful target for reasons that differ by modality. It is an absorption region for certain drugs and emerging modalities, including biologics and nucleic acid-based therapies, and targeted delivery there has the potential to enhance local efficacy while minimizing systemic exposure and off-target effects, come il Capsugel summary note. It also carries the highest concentration of Peyer’s patches, which matters for mucosal immune targeting, as the Nefecon imaging study reports.

Hitting it reliably is the hard part. The region has a dynamic physiological environment and a relatively short transit time, and the summary identifies reliable release there as one of the greatest remaining challenges in oral drug delivery.

The best available benchmark comes from a human imaging study of the Nefecon capsule, a targeted-release budesonide formulation approved for primary IgA nephropathy that acts on galactose-deficient IgA1 overproduction in Peyer’s patches. The study used test capsules reproducing Nefecon’s delayed-release function, with caffeine as a marker for capsule opening (it appears rapidly in saliva after release) and magnetic resonance imaging with black iron oxide to locate the capsule and visualize dispersion. In vitro dissolution confirmed the test capsules matched Nefecon’s release characteristics. In 10 Di 12 volunteers the capsule opened in the distal ileum; in the other two it opened just past the ileocecal junction. The authors present this as favorable against the high variability reported in other published imaging studies of delayed-release formulations targeting the gut.

That result is a distribution, not a point. Ten of twelve is a strong outcome for the field, and it still means a release specification written against a single nominal time is under-specified. If your target site is the distal ileum, the specification has to describe acceptable behavior across a transit distribution — including the tail of subjects who arrive late — and the assay panel has to be able to distinguish a capsule that opened in the wrong place from a peptide that degraded after it opened in the right one. Note also the scope: this is a 12-volunteer imaging study of a budesonide formulation, and its numbers should not be transferred to a peptide program as an expected success rate.

The Label Is Part of the Experiment

Every transport or biodistribution experiment on a peptide requires a way to see it, and the way you see it is a chemical modification. The common options are fluorescent dyes, radiolabels such as 3H, 14C or 125I, isotopi stabili, biotina, and various chemical tags. Each carries a different perturbation profile: a bulky fluorophore can dominate the surface properties of a small peptide, a radiolabel substitutes an atom, biotin adds a large hydrophobic-bicyclic ligand that can bind streptavidin-like proteins in unexpected places.

IL microfluidic intestinal-barrier work states the risk directly. Because the microscopy readout requires a fluorescent reporter, and because modification could potentially alter the transport behavior of biologics — especially for smaller peptides — the authors argue it is paramount to select dyes that minimally interfere with the physicochemical properties of the biologic, for example fluorophores shown to have minimal propensity for lipid membrane interaction. They cite work in which fluorophore labeling of a cell-penetrating peptide significantly altered the mode and degree of biomembrane interaction.

That is a measurement artifact with the same magnitude as the effect being measured. A labeled peptide that binds membranes more avidly than its parent will look more permeable in a cell model and less available in a luminal stability assay, and neither result reflects the unlabeled molecule.

La regola pratica: choose the label that minimally perturbs the property you are measuring, then confirm that labeled and unlabeled species behave comparably in the assay that matters before trusting the readout. For a permeability study, that means comparing transport of labeled versus unlabeled peptide in the same model. For a stability study, it means confirming the label does not change the degradation profile. For a biodistribution study, it means an orthogonal quantification method — LC-MS/MS of the unlabeled peptide, for instance — against the imaging signal. If the labeled and unlabeled species diverge, the label is the experiment, non il peptide.

Reading the Data: Which Assay Answers Which Question

A failed oral peptide experiment produces a symptom, not a diagnosis. The table below maps common symptoms to assays that can separate their causes. It is a starting framework, not a validated decision tree; the specific methods and acceptance criteria belong to your product and should be justified against Io Q2(R2) and your own development stage.

Sintomo

Assays that discriminate

Low plasma exposure after oral dosing

Poor capsule release; peptide degradation in gastric or intestinal fluid; bassa permeabilità epiteliale; adsorption to capsule or tubing; aggregazione

High impurity formation in the formulation

Chemical degradation (ossidazione, idrolisi, deamidazione); peptide–excipient interaction; physical state effects (solution vs. sospensione)

Forced degradation with fragment identification by MS/MS; excipient compatibility study at the intended storage condition; comparison of solubilized versus suspended API under identical conditions

Capsule opens in the wrong segment

Shell composition or coating thickness; curing parameters; transit variability

In vivo imaging with a position marker; in vitro release in a multi-compartment dissolution model; parameter study on curing temperature and humidity

Signal present but effect absent

Label artifact; assay interference; peptide inactive after labeling

Parallel unlabeled quantification (LC-MS/MS); orthogonal activity assay on labeled and unlabeled material

Batch-to-batch variability in delivery performance

Counterion or net peptide content differences; residual Of Peptides Factory acqua; stato di aggregazione; particle size

Counterion content by ion chromatography; net peptide by CHN or quantitative amino acid analysis; Contenuto d'acqua Karl Fischer; particle size distribution

The analytical vocabulary matters here. Purity is a relative area percentage by HPLC and says nothing about how much peptide mass is present. Identity is an orthogonal structural confirmation — mass spectrometry plus a second technique. Contenuto, o peptide netto, is a mass-balance quantity that accounts for counterion, acqua e solventi residui. Online Peptides Stability is a study design with defined conditions and timepoints, not a single measurement. Assay suitability is about format, matrix and controls. A single HPLC purity number cannot adjudicate a delivery failure, and treating it as if it could is one of the more common ways a program spends a quarter chasing the wrong variable.

Io Q2(R2), adottato 1 novembre 2023, is the relevant anchor for the analytical side. Its specificity/selectivity section calls for representative data — chromatograms, electropherograms, spectra — with relevant components labelled, and for investigation of suitable discrimination at an appropriate level, such as resolution of the two closest-eluting components in a critical separation. That is the standard a delivery-relevant method should meet: it must be able to see the degradation product you are looking for, not just the main peak.

From Synthesis Data to Delivery Decisions

The reframing is straightforward once the barriers are laid out in order. The certificate of analysis, il cromatogramma, la forma della controione, the solubility and aggregation data, and the stability timepoints are not release paperwork that follows a synthesis campaign. They are the inputs that constrain every downstream choice, and they should be specified with the delivery route in view from the start.

A workable sequence runs like this: Purchase Peptides

  1. Define the target GI site and the release window. Distal ileum, proximal small intestine, or stomach changes the shell, the coating, and the transit assumptions the specification is written against.

  2. Derive stability and permeability requirements from the sequence. Identify protease-susceptibility motifs, oxidation-prone residues, segmenti soggetti ad aggregazione, and the charge and hydrophobicity profile. These set the minimum stabilization chemistry and the permeability strategy.

  3. Select stabilization chemistry. Non-canonical residues, ciclizzazione, N-methylation, lipidazione, or ion pairing — each addresses a specific liability identified in step 2, and each is a synthesis decision with its own analytical consequences. Isotope Labeled Peptides Factory

  4. Select the formulation. Lipid phase, permeation enhancer, protease inhibitor, pH modulator. Confirm the peptide’s physical state in that vehicle and its compatibility with the excipients at the intended storage condition.

  5. Select the release system. Capsule shell, coating, or bilayer architecture, with the release specification written against a transit distribution rather than a nominal time.

  6. Define the assay panel that proves each step. Dissolution with site-specific sampling, stability-indicating HPLC, conferma dell'identità ortogonale, content and counterion, and the labeled-versus-unlabeled comparison for any imaging or transport readout.

The critical discipline is recognizing when a sequence change invalidates a prior formulation decision. Swap a residue to improve potency and you may change the charge, the aggregation propensity, the log P, or the oxidation liability — and the ion pair, the lipid vehicle, and the capsule release profile may all need to be re-derived. A sequence version that is not change-controlled is the most common cause of a formulation result that cannot be reproduced. Treat the sequence and its modification history as an immutable record, and re-run the affected formulation work whenever that record changes.

This is also where the two-tier GLP-1 study earns its place as a model. The authors did not hand a finished API to a formulation group. They changed the salt form to change partitioning, studied the physical state in the vehicle, identified oxidation liabilities from the sequence, and built the capsule around the formulation’s needs. The synthesis and the delivery were one program.

Where MOL Changes Fits

The framework above calls for an upstream partner that can produce the peptide and the characterization data with the delivery route already in view. Modifiche MOL is a peptide research and development organization that states it provides multiple viable research avenues for the advancement of peptide science, and its stated capabilities map onto the early steps of that sequence.

On synthesis, the company states it has mastered solid-phase, fase liquida, misto liquido-solido, inverse solid-phase and microbial fermentation methods, with customized sequences up to 200 amino acids and scale from milligrams to kilograms. On modification, it lists cyclization, pinzatura, glicosilazione, fosforilazione, PEGilazione, lipidazione, isotope labeling, fluorescent labeling and biotinylation among more than 300 modifying groups — the chemistries that appear in steps 2 E 3 Sopra. On quality, it states that production is maintained in a supplier-described Class 100 clean room from the first experimental step through packaging, with automated monitoring of sterilization, pressione dell'aria, umidità, temperature and dust particles, and that QC covers HPLC, SM, purezza, contenuto, endotossina, sterilità, pH and moisture with batch-specific data.

Two of those offerings are directly relevant to the measurement problems discussed earlier. Fluorescent and isotope labeling services are the same modifications that can perturb a peptide’s transport behavior; a labeling partner that reports the labeled species’ analytical profile alongside the parent gives you the data to check comparability. Counterion choice is a related lever — the company lists TFA as default, with acetate, chloride or ammonium salt exchange available — but the log P shift in the two-tier study, from −2.9 ± 0.3 for exenatide acetate to 0.9 ± 0.2 for its DOC ion pair, came from hydrophobic ion pairing, not from that kind of salt exchange.

To be clear about scope: MOL Changes states these as its own synthesis, modifica, scale and QC capabilities. It does not perform the capsule engineering, imaging studies or clinical work described in the sources above, and nothing here implies otherwise. The connection is narrower and more useful — the synthesis and characterization choices that determine whether a downstream delivery program has a workable starting point are made at this stage, and they are better made with the delivery route already specified.

Divulgazione: MOL Changes is a commercial peptide synthesis and modification provider and has a commercial interest in the quality and documentation standards discussed in this article.
